The program is conducted within GP surgeries. Diabetes specialistteams visit surgeries for full day clinics aiming to undertake ten (10)case conferences each day. On clinic days, the diabetes specialistteam consult patients with their usual GP and Practice Nurse.

The Diabetes Alliance Program Plus (DAP+) comprises an initial caseconference clinic (maximum of 10 patients per day), and a six (6)month follow up clinic with the same patients providing:

“We bring the Diabetes Specialist to you!”

DAP+ Virtual Case Conferencing model is an additionaloption for patients, GPs, and GP practices. Patients withdiabetes can be seen in a video consultation at their GPpractice with their GP and Diabetes specialists.

DAP+ Virtual Case Conferencing is a culturally safealternative that enables support from families or carers andeliminates access and transport difficulties for patients.

Clinicians can call the Endocrine and Diabetes Alliance Advice Line number and speak with an endocrinologist advanced trainee (AT) to obtain clinical advice in relation to diabetes treatment and associated care on the same day.
