General practice-based clinics are the cornerstone of the Diabetes Alliance Program Plus (DAP+).
DAP+ offers health professionals a number of services to improve the health outcomes and experience of care for their patients with diabetes.
DAP+ services include:
Case conferencing clinics between DAP+ specialists teams and GPs
A telehealth service to eliminate access and transport barriers for patients
A DAP+ Advice Line for urgent and non-urgent endocrine and diabetes advice to primary and tertiary care clinicians caring for patients residing in the Hunter New England region.
