The Diabetes Alliance Program Plus (DAP+) combines the specialist skills of diabetes educators and endocrinologists who travel to a patient’s general practice or Aboriginal Medical Service to work with the patient’s own general practitioner and practice nurse.

Data-driven quality improvement activity; report presented during lunchtime education of case conference days;
Accredited Continuing Professional Development for Doctors and Nurses with lunchtime education held for all staff. The Diabetes Alliance clinic is accredited with RACGP and ACRRM for three (3) Educational Activity hours, two (2) Reviewing Performance hours and two (2) Measuring Outcomes hours;
Case conferences – specialist teams consult patients together with patient, their GP, and practice nurse at the GP practice providing significant upskilling and collegial networking.
GPs are renumerated via MBS to organise and coordinate a case conference with multiple item numbers to cover time to support the patient and provide a management plan working closely with the specialist team.
For people with diabetes:
Care is provided by their primary care team in an environment they are familiar with.
One-on-one consultation with their primary care team and the specialist team to develop an individualised care plan.
Empowerment to manage own health in a supported team.
DAP+ has broken down geographical, financial, and traditional barriers between primary and tertiary care. By integrating and providing care in locations close to and trusted by the patient, the program has improved access to specialist care whilst simultaneously upskilling GPs to provide better care to all patients.
