It’s estimated that 100,000 people in the Hunter New England region of NSW are living with diabetes however 30% maybe yet to be diagnosed while their disease is advancing.

Established in 2015, The Diabetes Alliance Program Plus (DAP+) is a proven diabetes care model that aims to improve the prevention, detection, treatment, and self-management of diabetes in rural and regional communities.

DAP+ has supported over 4,000 patients living with diabetes

DAP+ has been adopted by over 800 health professionals across NSW

DAP+ has worked with 160 general practices across NSW
